KIDZPLAY A COMPLETED WAIVER IS REQUIRED TO PARTICIPATE AT KIDZPLAY. NO EXCEPTIONS, SORRY  In consideration of being allowed to enter into the play center and/or participate in any parties or programs at KidzPlay, the undersigned, on his or her own behalf and own behalf of the minor identified below, acknowledges, appreciates and agrees that:  Inflatable’s can be hazardous and dangerous. Activities require strenuous exercise and various degrees of skill, expertise, and can be dangerous. I understand that these activities can result in serious injury to children or anyone participating. I assume any and all risk and damage or injury while on the KidzPlay’s premises. I am aware of the risk, hazards and danger of personal injury, disability and/or death as a result of participation at KidzPlay. In consideration for the admission of my child, I hereby release, waive and forever discharge and covenant not to sue Hardin Express Inc. d/b/a KidzPlay or Hardin Rentals and its owners, agents, employees, directors, officers, trustees and all other persons or entities acting on its behalf, from any and all claims, actions, damages, liability, cost or expense including attorneys fees which are related to or arise out of or in any way connected to my child’s participation or use of the facility. By the execution of this agreement, it is my intention to assume all risk of injury and do hereby surrender and waive any rights to sue or exercise any legal right to seek damages against Hardin Express Inc. d/b/a KidzPlay or Hardin Rentals, its owners employees, agents, officers, directors and/or all other persons or entities acting on its behalf. I acknowledge that my child’s or minor’s participation in activities at KidzPlay is strictly voluntary. I hereby certify that I’m over 18 years of age; I have carefully read the foregoing covenant not to sue and acknowledge that I understand and agree to all of the above terms and conditions. Children cannot be dropped off or unattended. Parents/Guardians are ride operators and must make sure kids follow these rules: No  turning flips, due to possibility of neck injuries Do not enter or exit bounce standing up (crawl in on hands and knees or sitting only) Must be considerate of kids that are smaller Do not jump down slides (feet first only) Do not walk up slides Do not go down slide head first (feet first only) Not allowed in restricted areas around inflatable’s Kids should not push or pull on others Do not climb over sides of inflatable’s Do not jump off the sides of inflatable’s Parents and Children must obey signs and rules posted at the entrance of each inflatable (occupancy, safety rules, weight limits, etc). No food or drinks in the inflatable area. Parents must clean up behind their Children (example: food and drinks or bodily fluids). KidzPlay associates will assist. Please notify an associate to help. If any inflatable starts to deflate. Immediately ask children to exit the inflatable. Report it to the nearest KidzPlay associate. Socks only, no shoes. NO eye glasses, jewelry, belt buckles, or any sharp objects in pockets Please use common sense!
